Lifetime Performance Medicine is a physician-led longevity clinic in Los Gatos, serving Saratoga, Campbell, San Jose, and the wider South Bay. If you have had bariatric surgery, you already did the hard part. You made a major decision, showed up for it, and changed your life. What we hear from many patients across Silicon Valley, though, is a quieter worry that comes months or years later. The scale starts creeping back, or they simply do not feel as strong as they did, and they wonder how to hold onto what they worked for. This post is for you, and it comes with a warm and honest promise. Your results are worth protecting, and there is a plan for that.
There is a reason this matters coming from us specifically. Our founder, Dr. Richard Nguyen, DO, is a board-certified general and bariatric surgeon with Vanderbilt fellowship training and more than twenty years of experience, practicing in the Bay Area since 2007. Lifetime Performance Medicine is the longevity division of Lifetime Surgical, where Dr. Nguyen performs gastric sleeve, gastric bypass, and other bariatric procedures.
That means we understand your surgery from the inside. We know what the operation does, what it does not do, and where the real long-term work begins. Very few longevity practices in Los Gatos or the South Bay can say the same, because very few are led by a surgeon who has stood on both sides of the journey.
First, a reassuring truth. Weight regain after a gastric sleeve or bypass is common, and it is not a personal failure. It is biology doing what biology does. Your body defends its old weight through hormonal signals that increase hunger and slow metabolism. Over time, portion sizes can quietly expand, and the powerful early appetite changes tend to soften.
Life adds pressure too. Stress, disrupted sleep, and the demanding pace of work across San Jose and Silicon Valley all nudge weight and metabolism in the wrong direction. Understanding this is the point. When you know regain has real physiological drivers, you can meet it with a real plan instead of guilt.
Here is something the scale will never tell you. When people lose weight, especially quickly after surgery, they lose fat and muscle together. Muscle is the tissue that keeps your metabolism active, your body strong, and your later decades independent. Losing too much of it quietly sets the stage for regain and frailty, even if the number on the scale looks great.
This is why we use DEXA body composition scanning, available in our Optimization and Executive tiers, rather than relying on weight alone. DEXA shows us how much of you is fat and how much is lean mass, and it lets us track that over time. If we see lean mass slipping, we can respond with targeted nutrition and resistance training before it becomes a problem. Protecting muscle is one of the most important things you can do to protect your surgical results for good.
Significant weight loss changes your internal chemistry. Hormones shift, thyroid function can be affected, and nutrient absorption changes after certain procedures, which can leave you tired and struggling to hold your progress. These are the kinds of things that get missed when care stops at the surgical follow-up.
Our approach starts with comprehensive testing, including an 80-plus biomarker panel, thyroid and metabolic markers, and full hormone testing. Where hormone optimization is appropriate and the labs support it, we offer bioidentical hormone pellet therapy for men and women, always guided by testing first. IV therapy options, including recovery and immune protocols, are prescribed by our clinicians based on your labs, not handed out by default. Morgan Carmean, ARNP leads this ongoing wellness care, and you will see Dr. Nguyen twice a year as part of your plan.
Surgery is a moment. Keeping your results is a practice, and a practice needs a system. That is what our membership model provides. It turns scattered good intentions into a physician-built twelve-month plan that is measured, tracked, and adjusted as you go.
At the center is your Lifetime Body Score across six domains, along with biological age testing through PhenoAge. Instead of watching one number rise and fall, you get a full picture of your metabolic health, your body composition, your fitness through VO2 max testing, and your biomarkers, all revisited over time. Wearable data and functional testing round it out. When something drifts, we see it early and act, which is exactly how results are held rather than lost.
Foundations membership begins at $4,500 per year, with Optimization and Executive tiers built around more individualized needs, and a la carte testing is available as well. The point is not the tier. The point is that after everything you invested in your surgery, you deserve care that keeps protecting it.
Yes. Lifetime Performance Medicine is in Los Gatos and cares for patients from San Jose, Saratoga, Campbell, and throughout the South Bay and Silicon Valley.
Absolutely. It does not matter where your bariatric surgery was performed. Our longevity plan is designed to protect and extend your results from wherever you are now.
No. Weight regain has real hormonal and metabolic drivers. Our job is to help you understand them and build a plan that works with your biology instead of against it.
Our founder is a board-certified general and bariatric surgeon, so we understand your procedure and the long-term work that follows it in a way most longevity clinics cannot.
Every membership begins with the same baseline panel and the same two visits: an intake with Morgan, then your Body Score reveal with me. If you have been guessing about your health for years, the first step is to stop guessing.

Dr. Nguyen has practiced in Los Gatos since 2007. He leads every membership: the Body Score reveal, the 90-day plan, and the annual review.
